It's true that there are a lot of forms that need to be filled out, unfortunately it is unavoidable and mostly, they are intended to protect you.  A few are there so your insurance can be billed.

We are trying to make this as easy as possible and where possible you should only be required to enter information once.  If followed in sequence (begin with Telehealth & Financial) the forms are intended to remember, for instance, your name once you've entered it on one form and pre-fill it on other forms. In most cases, these forms will be automatically sent to our secure email. There is one form, the HEALTH INSURANCE CLAIM FORM that must be downloaded upon completion and emailed to us. Click on the name to fill out and download the form. A new window will open and when you have completed it and downloaded it, close the window and you will return to this page.
